You are responsible for self-certification for the first seven days of any illness including Sundays and your employer should provide you with the form SC2. Patients who are self-employed should complete the form SC1 that is available from the DSS. Thereafter you will need a doctor’s note (Fit Note). Please remember to make your return appointment well in advance if your fit note is to be extended.
Requests for sick lines do not constitute medical emergencies.
